Your registration is not guaranteed until receipt of payment. Payment (or purchase order) must be received before entry into any of the online modules. REFUNDS of the registration fee, minus a $15.00 administrative charge, will be made provided the cancellation is received via writing or e-mail one week in advance of the module beginning. NO REFUNDS will be issued after that time. Instructions will be provided after submitting this form as well as in your confirmation email. All correspondence will be done via e-mail unless otherwise noted on the registration form.
